About the Fund
Horizon Health’s vision is to positively impact the quality of life of older family members and neighbors living within the hometown they supported. Through careful planning and partnering, the fund will help ensure access to vital information, employment opportunities and healthy living choices all through life. The vision is a senior friendly environment where community members will maintain their active, healthy and independent lives. The fund will ensure the positive community impact for older adults will last for generations to come and help provide services for older adults allowing them to keep their independence. It will also support education and resources that will allow them to live their best lives in their golden years.
The Partnership in Life Fund (Horizon Health) is a Partner Fund of the Initiative Foundation. As a program of the Initiative Foundation, all Partnership in Life Fund (Horizon Health) contributions are professionally managed and stewarded for the benefit of the community now and long into the future.

Mail a Check: To make donations and payments more secure and to streamline processes, the Initiative Foundation partners with Bremer Bank.
