The Memorial Hospital Foundation supports health, wellness and related activities and projects in Isanti County and the surrounding area.
The Memorial Hospital Foundation is a Partner Fund of the Initiative Foundation. As a program of the Initiative Foundation, all Memorial Hospital Foundation contributions are professionally managed and stewarded for the benefit of the community now and into the future.

Mail a Check: To make donations and payments more secure and to streamline processes, the Initiative Foundation partners with Bremer Bank. Please write the check to Initiative Foundation-MHF, note the purpose of your contribution on the memo line, and mail to this address:

While the mailing address may be in St. Paul, the Initiative Foundation continues to maintain its headquarters in the heart of Central Minnesota at 405 First St. SE, Little Falls, MN 56345.

Fund History
The original Memorial Hospital Foundation was founded in 1992. The fund was endowed with the Initiative Foundation occurred in 2017 to better serve the communities and advisory team. The vision of the fund is to see a healthy and vibrant population in the greater Isanti County area!
Local volunteers know their neighbors, share their interests and are invested in the future of the community.
By promoting local philanthropy, the fund strives to strengthen the community by connecting people, resources and needs. The endowed fund is a charitable investment account in which the principal contributions are never spent; only the earnings provide a flexible, dependable funding stream to support grants for local projects.
Apply for a Grant
Building stronger communities doesn’t just happen.
It requires the efforts of many–each bringing their own unique talents, insights, strengths, abilities and willingness to make a difference. Helping people to do the most good in our community is what the advisory team strives to do through grant awards.
The Memorial Hospital Foundation welcomes and encourages grant applications from tax exempt, nonprofit organizations that are likely to have a substantial positive impact on health and wellness in the community.
Preference will be given to innovative programs that encourage volunteerism or community involvement.
Eligible applicants are those that serve people living and/or working within Isanti County and the surrounding area and include:
- 501(c)(3) nonprofit organizations,
- community groups operating through a mission-related fiscal host with the appropriate tax-exempt status,
- school districts, or
- local units of government.
Grants will be made only in support of proposals that fit the fund purpose and that benefit the community identified in the geographic boundaries described above.
The Memorial Hospital Foundation Advisory Board will periodically review the minimum and maximum grant size. Our current maximum grant size is $5,000.
The Foundation will not provide grants to/for the following:
- Grants, loans or reimbursements to individuals or businesses.
- Expenses incurred prior to receipt of this grant award.
- Replacement of government funding.
- Religious activities.
- Lobbying or campaigning for a candidate, issue or referendum vote.
- Any programs or projects that DO NOT directly benefit residents of Isanti County and surrounding area.
Note to applicant: Please allow 60 days between the grant deadline and the start date of your project to allow for grant processing. Grants cannot be made for expenses incurred prior to notice that the grant award has been approved by the Initiative Foundation.
